Beyond Pills: Precision Treatment for Pain
Joint and soft tissue pain (tendons, bursae) can be debilitating. When oral medications are no longer enough, joint injections emerge as a powerful and precise tool in the arsenal of modern orthopedics.
An injection consists of administering medications or bioactive substances directly into the source of the problem—a joint, bursa, or tendon sheath—to achieve rapid and localized relief of inflammation and pain.
I use injections for both therapeutic (pain relief) and biostimulatory purposes:
Corticosteroids (Anti-Inflammatories):
Mechanism: A powerful anti-inflammatory that reduces swelling and irritation within the joint.
Goal: To provide prolonged relief, allowing the patient to begin or resume physical therapy.
Hyaluronic Acid (Viscosupplementation):
Mechanism: A natural component that acts as a lubricant and shock absorber within the joint (primarily the knee).
Goal: To improve movement and reduce pain in cases of mild to moderate osteoarthritis.
What is PRP?
It is a concentration of platelets obtained from the patient's own blood. Platelets contain growth factors that are released when injected.
Mechanism:
PRP acts as a biostimulator, helping to initiate or accelerate the natural repair of damaged tissues.
Goal:
It is used to treat chronic tendon injuries (tendinopathies), ligament injuries, and, in certain cases, mild to moderate osteoarthritis, seeking a more biologic alternative.
Injections are an excellent intermediate treatment option for the following conditions that we manage in the office:
Knee: Osteoarthritis (cartilage wear), patellar tendinitis, bursitis
Shoulder: Rotator cuff tendinitis, subacromial bursitis
Hip: Hip osteoarthritis, trochanteric bursitis
Hand and Elbow: Epicondylitis (tennis elbow/golfer's elbow), tenosynovitis, trigger finger, carpal tunnel syndrome
The key to a successful injection is precision. The medication or PRP must be delivered exactly where the pain originates.
Controlled Technique:
I use anatomical landmarks or, in complex cases, perform the injection under ultrasound or fluoroscopic guidance to ensure optimal needle placement, maximizing the effectiveness of the regenerative or therapeutic treatment.
